How do I get notifications sent to my email address?

I am not receiving notifications to my email address.  They show up on the jot form website, but I am not getting notified when a form has been filled out.

I have checked your account and found it is associated with email address w**@********hhc.com, when you 'Test' the email notification it goes to the profile email address.
