For the form at the URL below, I need to create a way for a customer to elect to pay their bill now with a CC/Paypal, or pay with check later, and be able to submit/complete the form to receive an autoconfirm e-mail, as well as the form sending a notification to me, as well.  The form I have attached currently makes them complete the Paypal payment before sending a notification/autoconfirm....

    Hi,

You can create such a form easily in JotForm V4.

I have cloned your form to my side and narrowed it to show you key points.

Please look at my demo form below:

    https://form.jotform.com/70314795536966

You can clone this form to your side: How-to-Clone-an-Existing-Form-from-a-URL

Basically, you need to use Show/Hide conditions and Form Calculation Widget. After you create your form, you need to set Notification and Autoresponder Emails like below:

Please have a look at the following guides:

    Smart-Forms-Using-Conditional-Logic

    Setting-Up-Email-Notifications

    Creating-a-Form-Autoresponder

Finally, please make Clear Hidden Field Values 'Clear on Being Hidden'. (in Settings>Form Settings>Show More)

If you need any other details, please feel free to let us know.

    Thank you.
