Creating a form with multiple payment option.

    For the form at the URL below, I need to create a way for a customer to elect to pay their bill now with a CC/Paypal, or pay with check later, and be able to submit/complete the form to receive an autoconfirm e-mail, as well as the form sending a notification to me, as well.  The form I have attached currently makes them complete the Paypal payment before sending a notification/autoconfirm....

    Hi,

    You can create such a form easily in JotForm V4.

    I have cloned your form to my side and narrowed it to show you key points.

    Basically, you need to use Show/Hide conditions and Form Calculation Widget. After you create your form, you need to set Notification and Autoresponder Emails like below:
