I checked your form and I can see that the "Unique Submission" is set to "Check cookies only". This means that a user who already submitted a form cannot do another submission using the same browser. Here's a screenshot:
In the Form Builder V4, you can change this by clicking the "Settings" button at the top menu. Go to Form Settings, scroll down and you'll see the "Unique Submission" drop down. Please change it to "No Check".
