Embed code doesn't seem to be working

I added embed code to shopify and just got a black box on the page.

This is the code I used:

Did I do something incorrectly?

Try using your forms iFrame code instead:

https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Remove your current embed and add the iframe code.  If that still does not work, let us know and we will take another look.
