i just fixed it. had to clone one of my other forms and use the new cloned form. old form would not work. here it is still on one of my other sites.

I reviewed the website page and I can see the issue with the published form.
What I suggest you try first is to republish the form again on the website using instead its iframe publish code.
Let us know if this did not resolve the issue.
yes, thank you. I did discover that. apparently you changed the way iframes are handled. would have been nice to get a heads up.
