I'm looking to make a form that allows customers to buy products by filling in their information and shipping address and then pay by swiping their card through the Square card reader. Is this possible?
I also want people to be able to pick if they want to make a payment, or just answer a survey question. Is there a way that I can do this as well?

We have an integration for Square payments:

You can set the form up to allow for collecting payments or simply answering questions. Here is a guide on how to set up payment fields in addition to allowing for submitting the form without payment.

Instead of pay by check, your other option would be something like "fill out survey only" or something of that nature.

We've tested the integration, unfortunately, it's not working for the Square Reader. Your customers will need to key in their credit card details in the form in order to submit the payment. We have just released this integration, we'll inquire our backend team if the Reader will be supported soon.
