How Can I take subscription payments for a specific period of time (3 months)?

I'd like to use forms to allow purchasers to select an item and pay via Stripe or Authorize.net. I will feature approximately 12 options that the applicant/user can choose from. Based on their selection, they will be able to pay in up to 3 monthly installments.

Have you checked this guide on how to setup recurring payments: https://www.jotform.com/help/166-How-to-Setup-a-Recurring-Payment-Option? You are allowed to set the frequency or schedule or the recurring payments and the number of payments that can be done.
