Thank you page redirection error - Page not found

In our webpage (http://www.ywamepj.org/apply/truth-conference/) when people submit their application, after filling up all the fields... The page says... "page not found"...

I cloned this form and I found out that you are using Thank You Page Redirection. It means that when the user submitted the form, he/she will be redirected to the link you provided in the Thank You Page settings. 

The link is (http://www.ywamepj.org/apply/conferences-workshops-5/). I opened the link and it redirected me to this page:
