Once I have someone fill out my form online, I am not receiving a response email. What could be causing this?Page URL:

I checked your form (http://www.jotform.us/form/52915976212157) and I noticed that you have setup the email notification correctly.
Moreover, I did a test on your form to submit it (please ignore my submission). Can you please check if you have received an email notification of my submission?
