Next Button is not working

I open the web page, fill in the required info on page 1, but when click "next" the 2nd page doesn't load. Can you please fix? Not sure what I'm doing wrong.

Can you please try removing the Page Break and the Widgets on your form and re-add it back and see it this helps to solve the issue?

I have already clear your form's cache to refresh your form from our servers. After you have updated the fields on your form and the widgets, please also try clearing your browser's cache.
