Why is my form disabled?

    On the form creator my form says "Enabled," but on my website when I click the link to the form I get a message "Form not Found" "This form is disabled." Sometimes the form link works fine, and others I get that message.

    Hello,

    account is suspended due that your form "Leaf Peep M.O.W Order Form" you are requesting credit card information in plain text fields, as shown below:

    Please be aware that this is illegal because it's considered a phishing activity, which violates our Terms of Use, the only services allowed to collect  such data are those that are DSS/PCI certified.

    In light of this, it is a must to take any of the following options:

    1. Integrate your form with any of our Payment integrations
    Dwolla integration, is in particular a great choice for USA based business, it offers to accept payments with no fee per transaction.

    I have removed the guilty fields on the form and reactivate your account.
