    Hi,

I'm not quite sure if I understood your concern correctly. Would you like to have a feedback form placed on your website? If so, then you can do that by creating a form asking users for their feedback. You can find some templates regarding feedback forms on our Templates page here: https://www.jotform.com/form-templates/search/feedback

If you wish to embed the form on your website, you can do that by following this guide: https://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use. You can embed the form on different platforms or using different embed codes. You can also just get the form's direct link and share that instead.

If I misunderstood your concern, please do provide us more details.

If you are referring to show your submissions on your website, it is possible with two different methods.

One of them is submission view application and the other one is grid listing report. You can also customise the submissions you would like to show on your website with both methods.?

There is well explained answer at following thread.
