Why my form does not look the same in Firefox?

    My form looks correct in Chrome, but is messed up in Firefox. Why the difference?

    I have just checked your form in both, Firefox and Chrome, I can see it shows properly in both browsers, here are some screenshots about it.

    Are you still experiencing the issue on your end?

    Could you please test your form using incognito mode? That will load the latest version of your webpage and form. 

    You may also try embedding your form using the iFrame code instead, this guide will help you getting the code: https://www.jotform.com/help/148-Getting-the-form-iFrame-code
