How can I collect the same information (ie. Name, Title, Address, Email, Phone) for multiple nominees?

I am trying to make a nomination form. The challenge is that people can nominate either individuals or groups of individuals and I cannot figure out the best way to collect all of the information for groups.

I suggest you use the Configurable List widget. This widget allows you to dynamically add fields. Here's a helpful guide: How-to-Set-Up-the-Configurable-List-Widget.
