One of our colleagues cannot open the PDF reports that are generated when a submission is received. Not sure why it would be blocked for phishing. Please help. Thank you.

I checked your form. I cloned it and sent a test submission. I can download and open the pdf attachment on the notification email. The form ID that I checked is 70801291280449.
