Unable to upload image in the file upload field

Also, for uploading images, what is the max size?  We tried just a regular phone picture and it wouldn't take it.

I cloned your form and I found out that there's a limit in your File Upload field. The max limit is only up to 1024KB. The solution is to increase the max limit or turn off the limit file size. 

Select the File Upload field and then click the "Properties" button. Go to the "Options" tab and change the Maximum limit value. Please take note that this is in KB (kilo bytes).
