how can i change the form url to use mine ?

    Hello 

    please, I need to know how can I change the form jotform URL and replace it with my custom domain?

    additional how can I change my username?

    Or you can use a custom domain:

    You would need to purchase a domain name from a hosting provider (For example: Godaddy.com). You do not need to purchase web hosting (only the domain name)

    Then you need to log into your provider and setup a CNAME DNS record to point your domain to JotForm.
