Can you please be more specific? You can upload a screenshot if you like. We need to know what error message are you getting?
Also which form can you not access?

I have checked our records with your email address and unable to find any accounts registered. However, as I check the last edited form, I see that the form is associated with a different account candlerosp which is registered using a different email address.
If you are referring to this account and do not have the password to login, you may request to reset the password from the login screen so that the password reset link will be sent to the account registered email address.
