I used the following to embed the form into my wordpress website:
[embed]https://form.jotform.us/form/72155286891162[/embed]
The website is located here:
http://redoxcrossfit.com/founders-club-wods/
But for some reason the submit button isn't working. It just changed from submit to "please wait" but never actually submits that data. The form works fine when it is located on the jotform website.

Could you please try re-embedding your form by following one of these guides:

https://www.jotform.com/help/174-Embed-JotForm-in-Wordpress-using-the-Embed-Form-Plugin
If that doesn't work, please try with iFrame embed method:

I already tried all of these.
The wordpress.org version shows a blank page.
I can't try the plug-in version without contacting my web designer.
And the iFrame version only shows part of the form with no scroll bar. I even tried editing the iFrame to either add the scroll bar or increase the height but neither worked.
The only way I got it to work is by using the source code, but the formatting looks a bit sloppy, especially on mobile.

I checked your site (http://redoxcrossfit.com/founders-club-wods/) and I was able to see the issue you are reporting. Some fields in the form are not properly displayed on mobile view.
Can you try to re-embed the form on your website using this iFrame code only and see if the form will be displayed properly?
