My password is suspended.

Please reset my password as I am not able to login to Jotform. It says that the password is suspended. I have not tried to enter invalid password again and again.

I checked your account and found that it was suspended due to your form containing fields asking for username/password combinations. 

The collection of such sensitive information is prohibited here in JotForm as indicated on our terms of use (http://www.jotform.com/terms/).

We can reactivate your account but we will need to remove the fields related to the collection of these details.

I needed those fields because I was creating an assessment portal for my company. Cant it be left as such?

If no then you can remove that one field where it is asking for username and password and let the rest remain untouched. I have to check the form and deliver it as well.

Unfortunately, JotForm does not have the capacity to process logins. So there is presumption and including via the filters that there is an attempt to collect sensitive information from unsuspecting users. We cannot also give any exceptions to this as per our policies with security certifications and assessments.

I have now reactivated your account, but please take note that since you are also asking for passport number, although this is not purely illegal, some countries have a fair warning on providing such information.
