Text part of the form does not show up in the email notification

The last part of the form..the online waiver release form doesn't show up in the submitted emails back to me

I am not sure if i understood your question properly,but let me answer. I have taken the initiative of viewing your form and tried filling in all the parts,the best part is i reached the submit page. The page i was viewing is clients' form.You cannot get the reply of the message in that form.

When I get the sub mission email...the waiver statements are not there..just the signiture.
