How to create custom smart forms for healthcare?

    I would like to know if you are able to help me design a smart form for use in my dental office. We need a template to be able to complete a series of If/then type entries for each patient we see, and also be able to use multiple choice or drop down options for each section of our form. I also want to be able to view it in the format of an online app that progresses after each question is answered. Can someone contact me to go over more specific details?
