form not printing all information

I have a HR Application Form that has a disclaimer at the bottom we need to print and that disclaimer does not print, not does it show on the preview..

While viewing your submissions, there is an option to include headers and text in your submissions:

I have more information... Apparently they do not show up in the email of the submission.

By default headers text and images from the form are not included in email notifications or autoresponders.  If you would like to include them, they would need to be added manually to the body of the emails:
