I tried to attach a picture of the PDF this generated but the form fields and purchase order parts of the PDF are not visible.

However from what I understand you would like to have your form send email with PDF attachment that has all the fields including purchase order.

Hi I have done that but the result is that the form is not displaying even close to properly.  The whole purchase order piece is missing,

I am needing to have the form in PDF because I need to attach it to an order at the end of checkout.  That is where people are going to fill in the details of the order.  I am going to attach it as a digital download.  Therefore I need the PDF to match the form and I can't put it inline in any of the other publishing options.

Thank you for the clarification. From your elaboration the best approach would be to have an email notification with PDF attachments that has all the details of what the user has filled in the form. With this you can be able to attach it as digital download both on user email and the notification that will be sent to you.
