Apologies for the challenges you are facing. I have not understood your challenge and would like to kindly request if you could elaborate on what exactly the challenge is.
However from what I understand you would like to have your form send email with PDF attachment that has all the fields including purchase order.
To do so kindly follow the guide below.

Apologies for the challenges. As I had requested earlier would you kindly elaborate on what the challenge is and what you would like to achieve?
Do you want your form in PDF or do you want your form to send PDF submissions when filled? Kindly elaborate on this for easier understanding of the challenges you are having.
This is how I see your form from my end which is slightly different from what you have share in the attachment.

I am needing to have the form in PDF because I need to attach it to an order at the end of checkout. That is where people are going to fill in the details of the order. I am going to attach it as a digital download. Therefore I need the PDF to match the form and I can't put it inline in any of the other publishing options.

Thank you for the clarification. From your elaboration the best approach would be to have an email notification with PDF attachments that has all the details of what the user has filled in the form. With this you can be able to attach it as digital download both on user email and the notification that will be sent to you.
Below is how you can setup PDF attachment to email.
Kindly do let us know if this works for you.
