How can I add a Form to Facebook?

How can I add a Form to Facebook? I followed all these steps one by one and it says it's been added successfully but nothing shows up. Please advise.

If you see the following error while sharing your form on your facebook page:

Firstly, you need to have a group page, not personal(individual) timeline/profile to share your form on the facebook page.

So, please create a group page in your account and then, you will be able to add your form as a tab as you can see the example below:
