How do I remove a shared template?

I accidentally shared a template and it has my personal info in it.  How do I undo that?

You can unshare a form template in My Forms page. Here's how:

1. In My Forms, select the form and then click the "More" button. After that, choose "Share as Template" option.

2. In the Share Form Template gallery, click the "Unshare Form" button.
