All my forms but one automatically when I submit the form and receive it all have either the PDF attached in the email or the option to download the PDF from my email. I've checked all setting and options and not sure how to correct this one formPage URL:

I cloned your form to test it and it looks like the PDF attachment option is not currently enabled:
Check to make sure this option is enabled in your notifications settings. Once that is enabled there will be a PDF copy of the submission data attached to the email.
