I need to develop a patient intake process form where the client has to select their main symptom. I have 47 symptoms for them to choose from. Is there a limit of choices with jotform? What type of question format would you suggest that I use? thank you.

Hello,
You can add as many options as you want as multiple choice answer. If they need to select only one out of all options I would recommend Single Choice type of question ( radio button ). If they are allowed to select one or more I would recommend Multiple Choice ( check box ).

Thank you Onur, I was assessing Typeform but the 26-choice limit was a problem for us.
Is it possible to try your product for suitability before taking out a subscription?
Do you have a recommended template that makes your UI and UX similar to the look of the default style of Typeform? (I don't want to sound impertinent but I ask as this was already approved by the clinic as suitable for use).

We do not have a limit in the amount of choices you can add in a field.
