The credit card payment option has disappeared from my form. I have checked the revision history and account activity logs, however no changes are identified that caused this.
Any further help you can provide is much appreciated.

A regular Paypal integration has only one method of payment, and it is by redirecting the user to the Paypal page. There, the user has a choice to login or to click the other button to pay by debit/credit card (notice the grey button below the login button).
