Add a group of fields based on the number inputed from another field in the form

We're polling our guests to see if they need childcare,

Basically we ask "How many kids are you going to bring?"

From there, based on the number of children entered, I'd like to get the child's name , age and allergy info/medical conditions

Is this possible using the conditional logic?

This is possible with conditional logic only if you add all the fields first, and then setup associated conditions. Unfortunately, the fields cannot be automatically generated by the form.

However, the fields can be generated with some multiple entries widget fields. Fox example, Configurable List widget support sections with different field types, take a look at the next guide to see if this is something that might work for you.
