I need to create a Conference registration form with two payment options

Dear sir/madam,

I need to create a Conference registration form with two payment options:

-credit card (two fee options)

-bank transfer (two fee options)

Could you please give me information how to do this?

In order to receive online payments via credit card by the participants, do I obligatorily need to be registered in one of the available online payment services such as PayPal, authorized.net, etc?
