How can I create a form with a picture behind and organized spatially?

I have a diagram that I want to have in the background of the form and place fields spatially within designated places to be filled out.  The diagram is linked here.  I would like to put a few fields placed throughout the diagram.  Is this possible?

Yes, this is possible by injecting custom CSS code to customize the position of the fields. However, this may take a lot of time and/or custom CSS code to be injected into your form. When adding the custom CSS code, we also need to take consideration the different sizes of screens (i.e mobile devices).
