My form has disappeared

May I know the name of the form? I have checked your account you have 4 forms in it.

Also, you can check your trash if you thought you have deleted the form.

Then if you find your form you can restore it.

Hi Richie,Thanks for getting back to me.  I am Treasurer of Friends of the Dominican Republic.  As you have seen, we have a form called Dues and Donations which provides our supporters with several choices.  Yesterday, I went to edit the form.  I wanted to delete only the last choice which was called something like Univ Scholarship Pilot Project.  I don't know what happened, but now the form only asks for name, address, etc and does not contain any donation choices.  Pls advise.thanksJohn John D Evans 4512 Park Road Alexandria, VA 22312 703-354-9225 (H) 703-336-7218 (C) jequijote@verizon.net

Just click your form in your myforms then click More,

then choose Revision History,
