We need to add more fields to our form

Hello,

Our intern who set up our forms in Jotform is no longer with us. We need assistance or guidance with our Marketing Request Form.

1. We need to add additional agent on the form.

2. Along with their email and once completed the form will go to all parties. 

Basically and time edits are done we would like all parties included with the changes and more importantly, the agent should have 

To add another agent, it seems it would be done by adding another name field to the form for the third agent.  If you need to update who form emails are sent to, here is how to change that:
