Card Layout: Review/Submit button not working

I have a problem to "review and submit" on my form.  Neither it reviews nor submit. It let me go through all the questions until E-signature.  It doesn't let me sign and at the end of questionnaire, click on 'REVIEW & SUBMIT", the screen goes blank.  I have about 132 questions on the form.  Some pointed out the form is too long and heavily populated so takes long time to load.  But how long I waited the screen doesn't display anything.  Is there any solutions for that, other than cut down the series of questions shorter?

I was able to replicate the issue, it seems that it is due to widget Smooth Signature.

Can you please try using E-Signature widget instead. I was able to sign using the E-Signature.
