Salesforce integration: Problem mapping fields

Hello there,

Once I authenticate my login with Salesforce, I'm prompted to select the list that the form submitters will be added to. No matter which I select, Match Your Fields will not give me any options to match. Please see screen shot. This occurs if I select Account, Lead, or Contact.
