My form is showing a different name in the browser tab

I would like to create the same form as the one in the link below. Someone sent me the template that they used and I just went in and changed my name. I thought that this would work, but the tab in the browser when you open the form has her name. What template would this be so that I build a new form. Thank you for the help. John

That is the page title. You can change the page title in the form settings.
