Forms not working in Chrome

Good afternoon, 

Our forms are not working properly in Chrome. I am not sure if it has something to do with JS, but certain elements of the form are not showing up, and the submission button isn't working. 

The form with the drop-down elements not working (dynamic drown-downs) is an intranet page, so I cannot link it.

I tested your embedded form in Google Chrome and I was able to replicate the issue you reported. I also tested you standalone form and it is working properly.
