Jot Form's being blocked

    Hello, our IP is being blacklisted for all of our Jot Form uses, we are unable to view submissions in the past and also unable to have people submit new applications, we use it as a city to accept job applications as well as utility applications so we need this issue resolved as soon as possible.

    This may have happened since the account where the form http://www.jotform.us/form/60956144865162 was created has been suspended due to phishing suspicious, the account was suspended since you're asking for SSN and drivers license number, please note that asking for sensitive information goes against our terms.

    You will need to add a signature field and some sort of agreement in order to ensure your users agree to send you their information, the following widgets will help you with that:

    The changes as I have understood them to make us comply with your terms have been changed, can I have you check and confirm that our account should not be blocked again please?

    I have checked your forms and it seems like you have added some text to inform the users that they agree to provide you the information. 

    Your account should not be blocked again if your forms have the agreement text and as long as you do not ask for other sensitive that is not allowed such as credit card numbers or login credentials, there is not an exemption to collect this data.
