The form collapse and only shows half the line when you try to fill it in

Hi, I used your site to create a wholesale application for my website. The form loads alright, but when you try to fill in the information the form 'collapses' and only shows half the line you're trying to fill in... then when you try to tab to the next field, it appears to be stuck... but tabbing multiple times gets to the next field.... then the page collapses again... please advise how to fix, or I will have to switch to another service.

The script part controls the height of the form, and this is only useful if you have a multi-page form or has form collapse in it.
