Hi, I have been suspended how do I resolve this please?

    When I try to login i get this message Login -failed Suspended!

    Your account has been suspended due to the information you're requesting on your form http://www.myjotform.com/form/61012300319538, you're  currently asking for information that is considered as sensitive (National ID, Driver's Licence, Voters Card, International Passport).

    Hello Kevin,
    Many thanks for your quick response I really appriciate your help in trying to resolve this issue.
    We are asking for such information because as part of our programe, we will need to identify our subscribers and partners who signup / register to join any of our programmes.
    On www.lawquest-group.com website, you will find a few of such programmes for example "Register as a Probono Partner" Under this programme we register Lawyers with 10 years post call as probono partners and we asign enquiries from our subscribers to the partners. In order to participate as a probono partner your registration requires that we confirm that you are who you say you are, by providing us relevant identification such as Drivers Licence, International Passport Number, National Id, or Voters Cards. Please note also that we have not asked them to scan and submit such. Its purely for identification purposes.
    Secondly, we have a young lawyers training programme also on Lawquest website http://lawquest-group.com/younglawyers.html On this programme, we select and train young lawyers who meetup our criteria and then we attach them to law firms who are willing to support and train young lawyers. We also pay allowances during the training period. Now we need to properly identify these young lawyers by way of them showing their Id and School Certificates as evidence that they have graduated from law school and we also need their id's to pay them thier allowance during thier training.
    Thirdly, we also run an online publication named "Juriscope Legal Tips" www.juriscopelegaltips.com.ng were we have two types of subscriptions namely Basic Subscription & Premium Subsciption. Our subscribers recommend thier friends who signup for subscriptions. So you can see that we are not collecting this data for the sake of misusing it but for the purposes I have stated.
    I will adhere to your advice of connecting the Terms and conditions page as well as inform the users why we are requesting thier information which I belive is already known by them before they signup for any of the programmes.
    I hope this satisfies your criteria for me to reactivated. I also think i will need stepup my subscription to increase the submission allowance.

    Thank you for providing this information. 

    I have reactivated your account now without removing the fields so you can add the suggested elements to your form. 

    I can also see that you're using a corporate email (under your own domain) so as long as you have this and do not ask for other sensitive information, such as credit card numbers or login credentials which are not allowed at all, your account should remain active. 

    I already reactivated your account and whitelisted your IP address, if you have questions, please  let us know.
