Missing fields when published to GoDaddy

I think that your application is great and easy to use.  I am experiencing a strange problem with my form (https://form.jotform.com/80765480684164) located at https://heatison.shop/request-a-quote. For some reason, my long text field and upload documents field are not showing up in the published form.  They show up in Preview mode, but not live mode.

I checked the direct form link and the fields are showing properly. Upon checking your website, I was able to replicate the issue. The fields are not showing properly.
