    My account was recently autosuspended. Need to resolve this issue asap. Thank you!

    Your account got automatically suspended by our anti-phishing filter tool. As per our terms of use, collecting sensitive information is not allowed here in JotForm.

    I have removed the guilty fields and re-activated your account so you can use your forms immediately.
