When the form filled out online is submitted, I don't see subtitles between the sections

I created a form on your site, entitled ''Form Truck Driver Application''. On this form, I have subtitles such as General Information, License Information, Emergency Contact, etc. They are in the actual form but when someone submits an application online, I don't see form subtitles (see upload). Can you help me make my subtitles to be seen on the submitted forms as well? Thank you.

For any headers and text you would like included in the email notification, you would need to add them manually.
