Form showing as disabled, why?

I have recently sent out a form, which I received a few submissions and then none. I thought this was weird and a member of my audience sent me an email saying the form was disabled. They can answer the questions through the form (2 pages), but when they hit "submit", it says that the form is disabled. I have not disabled the form and I've even disabled it and then enabled it again. 

If you could let me know how to fix it, that'd be great! I would also like to avoid creating a new form, if possible, as I've already sent out the link to my audience.
