I created a form and submitted it but now it's disappeared

I got a URL but when I used the link in an email (sent to myself to check it) all it did was direct me to the 'start a form' page and not my created form.

What have I done wrong?

I cannot even give you the URL now as that's disappeared!

I spent ages on it as well.... :-(

It seems the form you are referring to was deleted.  If it was not fully purged from the trash, it can be restored from there.  Here is more information on how to restore it:
