I was able to correct the headers so that import of submissions comes in.  However, the date is still coming in as a number after having applied the steps outlined in the following

    https://www.jotform.com/answers/789817-Import-Submisisons-How-to-import-dates-from-a-spreadsheet

    did the following:

    - changed the cell format to yyyy-mm-dd

    - saved the file and imported

    - changed the date to text (which turned to number)

    - saved the file and imported

    - changed the date number to text

    - saved file and imported

All came back with number once imported.  See notated screenshot below

I am testing the Spreadsheet to form widget with a cloned version of your JotForm. Please allow me sometime to test it and get back to you with relevant information.

    Thank you for your patience.

Since Date field is not supported with the Spreadsheet to form widget, you may consider calculating the date to epoch time number on the spreadsheet. Since the system uses epoch (Unix time) you may consider using the calculation from 01-01-1970 to the date needed and then send the number to the form. This means, for the date 6th of May, 1970, it would be 125 days which you may calculate on the spreadsheet and then send it to the form field.

Please check the form below by entering the Unique code as '2'.

Here is how my spreadsheet date is calculated.

What is meant by epoch and how is this calculated? I have a lot of submissions.  Not sure if you are saying to go to each of the submissions and calculate what their epoch time is.  If so, could you describe a practical way of doing that as I have too many submssions in my database.

I assume that you are pulling the information from a spreadsheet uploaded to Spreadsheet to Form widget to the form fields and you are working with the form https://www.jotform.us/form/82265350984160. You may have an additional column to your spreadsheet for performing the date calculation so that it should be pulled correctly to the form. If you can share us the spreadsheet that you are working, we can help you how to get the date calculated.

We will wait for your response. Thank you!
