I've built this form for use in an employee benefits enrollment however it will not update the output PDF with the saved updates I've made in the settings.
Also, how do I include the agreement statement made with the widget on the PDF output?

I think you may be referring to the static text not being included in the PDF report, if that's the case you may try enabling the show headers and texts option in the submissions settings to include it.
