Can you create a voting form with assigned values for 1st, 2nd and 3rd place?

 Can you create a voting form with assigned values for 1st, 2nd and 3rd place?

i.e. We have a panel of judges who vote on several award categories. Each judge selects 1st, 2nd and 3rd choice in each category. 1st=5 points, 2nd=3 points and 3rd=1 point.

Can we do this in jotform? if so, how?
