My form works perfectly in IE, Chrome, Firefox. iphone shows perfectly, too. But Safari is showing bullets next to most elements of the form while working in Windows OS- testing in Safari browser.

I've tried it on Safari 11.1 and it doesn't show any bullets on your form.
Here's what I see from my end.
Would you mind sending us a screenshot so we can see how it looks from your end? Looking forward for your response.

Thanks, Jed. I figured it was my outdated windows-restricted Safari (5.1.7), so thanks for confirming!
As long as it works for the typical Mac user running any version that is relatively updated, I'm happy!

I also tested your form in safari browser and your form seems to display correctly. It should work correctly for your users as well.
